You have said yes, so this is the last piece of admin before work begins. It takes about ten minutes and your answers are saved in this browser as you go, so you can stop and come back.
Not here, not in the notes field, not anywhere. We will send you a secure link for credentials once the agreement is signed. Any password submitted here should be considered compromised and changed.
Nothing on this form should contain patient names, records or images. If a question seems to invite it, it does not; leave it out and tell us on the call.
Do not stall on anything you do not know. "Not sure" is a perfectly good answer and it is often the most useful one, because it tells us where the documentation gaps are.
